Jane skualifikuar: 1) Ment-Farma shpk, Nuk permbush kriteret e vecanta per kualifikim sipas DST. a) Ne lidhje me pervojen e mëparshme ne pune, operatori ka paraqitur vetem nje kontrate, por nuk ka paraqitur nje vertetim te leshuar nga nje ent publik ose nje fature tatimore per te vertetuar pervojen e mëparshme sipas kërkesave te pikes 2,3 te Kapacitetit teknik te shtojces nr. 9 te DST. b) Forme doza e deklaruar ne formularin e ofertes (shtojca nr.1) nuk eshte e njejte me ate te kerkuar ne dokumente te tenderit. c) Mungon certifikata e rregjistrimit te barit per tregtim ne RSH. Ka paraqitur nje certifikate regjistrimi per pajisje mjekesore. d) Nuk paraqitur autorizim nga MAH per tregtimin e barit ne RSH. |
